Insurance Claim Application Process
If you have medical insurance, accident insurance, or a reimbursement-based policy and would like to file a claim related to physical therapy, please follow the process below:
🔍 Confirm Your Policy Coverage
Before treatment, please confirm the following with your insurance company or insurance agent:
✅ Whether your policy covers the physical therapy services you will receive, such as rehabilitation after an accidental injury or treatment for a chronic condition.
‼ Important: Confirm that your policy covers self-paid treatment provided by an independent physical therapy clinic, rather than only self-paid physical therapy offered under a National Health Insurance clinic.

📄 Documents Required for Your Claim
At your first appointment or during the course of treatment, please tell us that you intend to file an insurance claim and provide the name of your insurance company and the items you plan to claim.
Please prepare one or more of the following documents, depending on your insurer’s requirements:
✅ Diagnosis certificate: Please obtain this from a general medical institution. Fees are determined by that institution.
📤 Documents Provided by the Clinic After Treatment
✅ Medical-record summary or treatment plan, if required.
📬 Submit Your Documents to the Insurance Company
Submit the documents to your insurance agent or insurance company. If additional documents are requested, please contact our clinic again and we will assist you.
⌛ Wait for the Insurance Company’s Review
The insurance company will review your documents. Processing times vary by insurer and generally take approximately 1–4 weeks.
💰 Claim Payment
Once the claim is approved, the insurance payment will be deposited into the insured person’s account. If there is a dispute, please contact your insurance agent or insurance company directly.

Lienbang Physical Therapy Clinic is not an insurance agent and cannot guarantee that a claim will be approved. All decisions are subject to the terms and conditions of your policy.
Keep all treatment receipts and records in a safe place to prevent loss.
Do not ask the clinic to issue documents that do not match the treatment actually provided. Doing so may violate the law and affect your insurance record.
For treatment involving multiple body areas or a long-term course of care, please discuss the scope and contents of available certificates with your physical therapist in advance.
